I've pressed 'post reply' instead of 'reply' myself - it's not clear that 'reply' generates a reply within the thread while 'post reply' generates a new thread within the forum. The clue for me is that if you're trying to reply to an existing subject and you're asked to provide a Subject for your post then you have pressed 'post reply' and are instead starting a new thread.
